The purpose of this position is to provide direct care to residents, under the supervision of a licensed nurse, in accordance with facility policies and procedures and report resident needs and concerns to a licensed nurse.
Qualifications:
Must have current license.
License must be free of any encumbrances Must not have any disqualifying findings barring employment as stated by State or Federal regulations.
Delegation of authority:
Provide care to residents following facility policies and procedures.
Report residents' needs and concerns to a licensed nurse.
Primary Functions and
Responsibilities:
Follow written and oral directions.
Follow established performance standards and perform duties according to nursing service policies and procedures.
Assist new employees in following established facility policies and procedures.
Identify resident problems and concerns and report them immediately to a charge nurse or a licensed nurse.
Complete assignments timely, completely and accurately.
Conduct resident rounds as assigned.
Initiate corrective action as necessary and/or seek assistance of a licensed nurse.
Identify safety hazards and emergency situations, and initiate corrective action.
Attend all in-service classes as assigned and complete assignments.
Assume personal responsibility for following facility procedures related to control of equipment and supplies within the unit.
Document incidents and/or unusual problems according to established facility procedures.
Participate in interdisciplinary team and other facility meetings as assigned.
Participate in the development of an individualized plan of care of assigned residents.
Review care plans and perform nursing care as outlined.
Document in the Point of Care Kiosk the care and treatment provided to the resident and the resident's response or lack of response to care provided.
Documentation is to be completed at the time care is given and all documentation must be finalized at the end of the assigned shift.
Maintain confidentiality of resident and facility information.
Report changes in residents' conditions to Charge Nurse or Nurse Supervisor.
Follow facility procedures for admission, discharge and transfer of residents.
Complete documentation accurately, timely and following facility policies and procedures.
Listen to resident and family complaints and report problems to Charge Nurse.
Assure residents have call lights at hand and answer call lights promptly.
Assure residents are given adequate assistance with personal and oral hygiene needs following facility's policies and procedures.
Assist residents with activities of daily living as documented in the residents' care plans.
Participate in the implementation of the individualized plan of care of assigned residents.
Assure residents are given adequate assistance with meals, nourishment and transportation to meal service area(s).
Ensure that all residents that have identification devices are present and functioning.
Consistently work cooperatively with residents, charge nurses, treatment nurses, restorative nurses, other nursing assistants, physicians, families, consultant personnel and ancillary service providers.
Observe all facility safety policies and procedures.
Come to work in a clean, neat uniform and consistently present an appropriate professional appearance.
Come to work as scheduled and consistently demonstrate dependability and punctuality.
Assume accountability for data contained in the employees' handbook.
Perform other related duties as directed by his/her supervisor.
Perform all duties assigned in an effective, timely and professional manner.
Assume Quality Assurance duties as assigned.
Observe infection control procedures.
Follow Residents' Rights policies at all times.
Be responsible for the safety of residents under his/her supervision.
Ensure that residents Right to Privacy and Dignity are observed at all times.
Accept assigned duties in a cooperative manner.
Has completed the state-approved Nursing Assistant Training Program.
Completes yearly 12 hours in-service programs as designated by federal government.
Attends annual training in dementia management and patient abuse prevention.
Uses only authorized abbreviations established by the facility when recording information.

code and password for accessing resident/facility information and promptly report suspected or known violations of such disclosure to the Charge Nurse.
Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Charge Nurse.
Maintains professionalism at all times, does not discuss personal problems with residents and ensures that communication while providing bed-side care is appropriate and beneficial to the resident.
Ability to work other shifts as needed.
Complete all duties assigned by his/her supervisor.
